Five forks on a transparent glass pane with a sheet of copypaper sticked to it with an adhesive tape. I chose to stick it from below instead of just putting it on the glass so that I did not lose the reflections which are crucial here. The only purpose of the paper then, was to make the light as even and smooth as possible.
The blue color of the forks is not a job of gells but of the WB which was set to tungsten (this is also why the below light is kind of pinkish rather than deep red).
Anticipating your questions - there's no Photoshop work here at all.
