Primary light was placed in front of the camera and slightly above with a small amount of diffusion (florescent ceiling panel). To the left of the camera and behind the subject (a little bit above), a single light for highlighting the edge. To the right of the camera and behind the subject (about face/torso level), two lights for a stronger raking light.

Colour and tonal adjustments in Photoshop, with minor light flare enhancements.
